The total cost includes all lodging, breakfasts and dinners, selected sightseeing, ground transportation, transfers, tournament entry fees, staff coaching and chaperoning, and liability insurance. NOT included in fees: Roundtrip airfare from team's origination point, lunches, passport/visa fees, laundry, medical expenses, optional activities, personal spending money, and extra baggage fees.

The above price is based upon projected fees for the summer of 2006, and may be revised based on changes in value of the dollar or inflation rates.

1. Payments shall be made as follows:
1. $1000 deposit to be included with this registration form.
2. Half of the balance no later than March 1.
3. Remainder no later than April 20.
4. Participants who enroll after May 1 are required to remit payment in full with the registration.
5. If final payment is not received by May 20, participant shall be canceled and refunds made as stated in Section 2.
2. Cancellation Policies
1. Prior to March 1: TENNIS: EUROPE will refund all fees paid with the exception of half the initial deposit of $1000.
2. Between March 1 and April 1: TENNIS: EUROPE will refund all fees paid with the exception of the initial deposit of $1000.
3. April 1 thru June 1st: Refund of 50% of all fees less any nonrefundable penalties of the airlines, travel agency, railroads, hotels or other suppliers of services or a minimum of $1500 whichever is greater. We recommend that cancellation insurance be purchased to cover these expenses.
4. After June 1st it is up to the discretion of TENNIS: EUROPE to refund any fees.
5. ln the event that a suitable replacement is provided by the canceling participant, at the sole discretion of TENNIS: EUROPE, all fees shall be refunded except $1000 and any railroad and airline penalties, or non-refundable fees.
6. TENNIS: EUROPE has sole discretion as to refund once the trip has begun. No refund will be made for temporary absence or voluntary withdrawal or if player is sent home for violating any of TENNIS: EUROPE's rules.
7. Parents must request a refund within 30 days of withdrawal and sign a release before refund is made.
8. We strongly recommend that cancellation insurance be purchased to cover the cost of the trip in the event of a cancellation.
3. Deviations of Flight Itinerary:
   It is strongly recommended that players travel with their teams from and to New York. If a participant wishes to make alternate flight plans, the following provisions shall apply:
   1. lf TENNIS: EUROPE makes the flight arrangements, there will be a $150 charge each time a change is requested, and participant will pay any additional airfare charges.
   2. If TENNIS: EUROPE does not make flight arrangements but participant is to be met at the airport nearest to the first tournament or escorted to the airport at the end of his/her trip, there will be a $125 charge per escort service.
   3. If participant wishes to return to his/her home city at any time other than our scheduled return flight, such participant shall be required to pay for the cost of such return if the TENNIS: EUROPE airline ticket cannot be used.
4. Responsibility
   1. TENNIS: EUROPE assumes responsibility for the participant upon arrival at the airline check-in at designated team origination airport and concludes that responsibility at baggage area or U.S. Customs Inspection area upon return to New York area airport.
   2. If participant joins or leaves his/her team at any time other than the specified beginning and end of the trip, said responsibility will be assumed and ended at the airport closest to the first or last tournament site, or at the tournament site, at a date and time stated on TENNIS: EUROPE's "Deviation of Flight Itinerary" form.
5. Participants must have full Major Medical Insurance and provide proof of same at least 30 days prior to departure. Participant should bring his/her insurance card and claims forms on trip. TENNIS: EUROPE is not responsible for doctor, hospital or pharmaceutical costs or travel expenses relating thereto. in the event that TENNIS: EUROPE provides medical payment for participant, parent/guardian will reimburse TENNIS: EUROPE for same upon notification of such expenditure.
6. Failure to make full discloure on medical form may result in player being sent home without refund.
7. TENNIS: EUROPE assumes no responsibility for damage, expense or injury caused by delays, or negligence of any company/vendor used by TENNIS: EUROPE or by reason of illness, strike, acts of terrorism, war, weather, or other events beyond the control of TENNIS: EUROPE. It acts in the capacity of agent only in matters of transportation or contracting with vendors. TENNIS: EUROPE reserves the right to make any changes in itinerary that it may deem necessary.
8. The undersigned agrees to indemnify TENNIS: EUROPE, its agents and employees, and hold them harmless from any financial liability or obligation which participant incurs. TENNIS: EUROPE will not be responsible for injury or damage to any person or property caused by the participant while on a TENNIS: EUROPE trip.
9. It is understood by the participant that this is a supervised trip and that trip standards and rules must be observed, that TENNIS: EUROPE shall have the right to remove a participant from the trip if it is deemed the participant's and/or participant parents' acts or conduct is detrimental to or incompatible with the intent, harmony, comfort or welfare of the trip as a whole, or for failure of participant to maintain standards or comply with the rules. Any additional costs or expenses resulting from such removal of participant shall be borne by participant and trip fees are not refundable.
10. TENNIS: EUROPE reserves the right to use photographs, movies, videos of and/or correspondence with participants in publicity materials. The participant and his/her parents/guardians hereby waive and release any rights to such materials that may be obtained during participant's activities with TENNIS: EUROPE.
11. Acceptance of this application and deposit shall be subject to satisfactory character and ability references. If such references are not received or are unsatisfactory, TENNIS: EUROPE reserves the the right in its sole discretion to reject this application and to refund the deposit.
12. It is agreed that any dispute or cause of action arising between the parties whether out of this agreement or otherwise, can only be brought in the county court in Fairfield County, Connecticut and shall be construed in accordance wth the laws of Connecticut.

     I understand that as a participant, my child will be required to abide by TENNIS: EUROPE's rules as well as the laws of the countries to which he/she travels. The purchase, use, possession or sale of illegal drugs is absolutely prohibited as is drinking of alcoholic beverages, smoking or improper behavior. TENNIS: EUROPE may dismiss my child and send him/her home at my expense and without refund for violation of TENNIS: EUROPE's drug, alcohol, tobacco or behavior policies or for conduct incompatible with the harmony, comfort or welfare of members of his/her team. I authorize TENNIS: EUROPE, in its sole discretion, to send my child home by commercial airline at my expense. In the event that my child is sent home at any time there will be no refund.

Gentlemen:

      The undersigned are either the natural parents or legal guardians of the above-named minor who is enrolled in Martin Vinokur's TENNIS: EUROPE, Inc. program. In the event medical or dental emergencies arise necessitating medical or dental treatment to the said minor, we hereby give you full power and authority to do and perform all and every act and thing whatsoever to all intents and purposes as we might or could do if personally present with full power or substitution, including, but not limited to, the signing of any and all consents requisite or convenient to obtaining medical, dental or hospital treatment for such minor. You may rely upon the recommendations and opinion of any medical practitioner, dental practitioner, or agency furnishing hospital services in the event they advise you that such minor requires such medical, dental, or hospital treatment on an emergency basis.

      It is mutually agreed that this authorization shall be irrevocable, and that any medical practitioner, dental practitioner, or agency furnishing hospital services may rely upon your executing all authorization on our behalf.

      It is further mutually agreed that you shall use your best efforts to notify us in the event of such medical, dental or hospital-type emergency.
